A match shows a CMS assignment; it does not establish coverage or the units to submit. How to read the fields.
CMS’s October 2026 ASP crosswalk links drug identifiers to HCPCS billing codes for ASP-based Medicare Part B payments. This copy covers October 1 through December 31, 2026. It is not a complete list of US drugs or NDCs, and it does not determine coverage or the units to submit on a claim.
How to read a match
Each result keeps the identifier, HCPCS code, drug name, labeler and HCPCS dosage text from the same CMS row. One identifier can have more than one row; check every match against the drug, package and applicable billing instructions.
- HCPCS dosage
- The dosage descriptor attached to the HCPCS code in the source. It is separate from the package size.
- PKG SIZE / PKG QTY
- CMS describes package size as the amount in one item and package quantity as the number of items in the NDC. Values below a result keep the source’s exact field names.
- BILLUNITS / BILLUNITSPKG
- These are the two CMS billable-unit fields, displayed as supplied. The source introduction distinguishes units per package from units per 11-digit NDC. Use the original file and your applicable billing instructions to interpret the values; this tool does not calculate a dose or claim quantity.

If the lookup finds no match
No match means the exact identifier is absent from this copy of the October 2026 ASP file. It does not mean the NDC is invalid, the drug is not covered, or no HCPCS code applies. Check the identifier and the file for the date and payment context you need.
The CMS ASP pricing files page lists quarter-specific crosswalks and revisions. The October ZIP also contains separate AWP, OPPS and PrEP files; those rows are outside this lookup. Medicaid and commercial requirements need their own payer sources. Use the payer-document directory to locate them.
